Check the box which concerns the intentions of your purchased goods, add your name and signature on the bottom of the box.

Get the form stamped and signed by the organization you work for.
Box 5A
Fill out the store details including the VAT number.
Box 5B
Fill the available cells with the required info like the invoice number, quantities and values. Also include the total amount of your purchases.
Box 6
This will be filled by the ministry of finance during stamping.

Leave this box empty, easy.
